A 7 year old child with cerebral palsy has been evaluated by an OT and makes several short and long term goals. After the OTA completes the next session and notes are completed, what should the OT do?
A. Read the OTA’s notes after 10 sessions with the patient
B. Review the notes and cosign if in agreement with all of its contents
C. Rewrite the notes since only OTs can write notes for a patient
D. Do nothing since it is not a requirement to co-sign OTA’s notes

B) It is highly recommended that notes be co-signed by an OT since these notes are subject to subpoena.
