An OT has been assigned to work with a patient who recently had a CVA. Evaluation results indicate that the patient requires moderate assistance for transfers and does not have functional motor control of the affected dominant upper extremity. Patient refuses assistance with ADL’s and denies having any problems. How should the OT best document the patients response?
A) “Patient requires maximum assistance for grooming activities”
B) “Patient requires moderate assistance during transfers.”
C) “Patient is in denial about current physical impairments.”
D) “Patient will benefit by compensating to use his non-dominant extremity for ADL activities.”

Its asking documentation by OT forthe response of patient.Here response is definitely a-a denial but OT should document it in such away to get reimburesement .D option is objective towards attainment of goal for this patient .
The documentation that patient is in denial state will cause the insurance denial .

I am confused. I dont understand how answer D is the patient”s response. I understand why c is incorrect but it seems as answer D is the therapist’s response and recommendation, not what the patient is communicating. ????

The question is asking for the patients response. ”Denial” is a subjective statement whereas answer D) is a statement of fact.
